Job Description / Responsibility
  • PATIENT INFORMATION AND RECONCILIATION.
  • To check and give input of the patient’s information, allergy information, Medicines Reconciliation and
  • compare the data with current medication to minimize the therapeutic duplication and drug-drug interaction.
  • PRESCRIPTION SCREENING: REVIEW EACH PRESCRIPTION WITH ALL RELEVANT PATIENT INFORMATION TO HANDLE AND INDIVIDUALIZE DRUG THERAPY ACCORDING TO-
  • The appropriateness of the drug, dose, frequency, and route of administration;
  • Therapeutic duplication;
  • Monitor rational use of Restricted Antibiotics as well as check patient’s Culture Sensitivity Test, Serum Creatinine, Creatinine clearance
  • Report to PRD department if more than 3 (three) antibiotics use concurrently
  • Monitor rational and safe use of High Alert and High Risk medicines.
  • TRANSCRIBING / INDENTING DRUG ORDER:
  • To ensure safe indenting and monitoring of medications in the organization. Indent the medicine for 24 hours.
  • And if there is any medication errors inform the respective consultant through Clinical Care Coordinator to
  • solve the error and also fill the Medication error form.
  • DISCHARGE PATIENT MEDICATION:
  • Check the patient pre discharge planning before processing for discharge patient medication and indent it as
  • per the instruction/ advice in discharge summery, return of unused drugs if doctor change or stop the drug.
  • TRAINING: Involves in the training and education of medical, pharmacy and nursing personnel in the patient
  • care areas.
  • WARD ROUND: Participate in ward round, taking patient histories as required; makes initial clinical
  • ASSESSMENTS AND CONTRIBUTING TO THE TREATMENT DECISION-MAKING PROCESS.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards

  • Patient box checking to monitor the medicine administration at right time as well as administration error.
